Indications/Uses
Management of HTN, angina pectoris, cardiac arrhythmia & MI.
Dosage/Direction for Use
HTN Initially 50 mg daily. Max: 100 mg daily. Angina pectoris Initially 50 mg once daily. Max: 100 or 200 mg daily. Cardiac arrhythmia 50-100 mg daily. MI 5 mg atenolol IV for 15 min, followed by 2nd IV dose of 50 mg for 10 min. Continue w/ oral dose 50 mg after 12 hr & then 100 mg daily for 6-9 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Severe peripheral arterial circulatory disturbances; cardiogenic & hypovolemic shock; CHF; hypotension; 2nd or 3rd degree heart block; sick sinus syndrome; sinus bradycardia; significant right ventricular hypertrophy; bronchospasm; allergic disorders; untreated pheochromocytoma; metabolic acidosis. Right ventricular failure secondary to pulmonary HTN. Anesth w/ drugs that produce myocardial depression (eg, ether, chloroform, cyclopropane).
Special Precautions
Avoid abrupt discontinuation of therapy in patients w/ CAD. Discontinue use if cardiac failure continues despite adequate treatment. Bronchospastic diseases; diabetes & hypoglycemia; thyrotoxicosis; Prinzmetal angina; peripheral circulation; 1st degree heart block; severe renal disease. Systolic BP <120 mmHg & 1st degree AV block. Patients w/ atrial fibrillation after MI may be digitalized prior resuming to therapy. History of anaphylactic reaction. Pregnancy & lactation. Elderly.
Adverse Reactions
Fatigue, malaise, lupus syndrome, sclerosing serositis. CV, GI & hematologic disorders. Angioedema, urticaria, fever combined w/ aching & sore throat, laryngospasm, resp distress. Elevated liver enzymes, AST, BUN, serum creatinine & bilirubin; hypoglycemia. Ataxia, leg pain. Nervous system disorders. Impotence, Peyronie's disease; wheezing, dyspnea, bronchospasm, pulmonary emboli, asthma, nasal congestion; psoriasiform skin reactions, exacerbation of psoriasis, reversible alopecia, skin & erythematous rash; visual disturbance, dry eyes, conjunctivitis, tinnitus, otitis, renal failure.
Drug Interactions
Anesth (eg, methoxyflurane). Potentiate effect on atrial conductime time & induce -ve inotropic effect w/ disopyramide & amiodarone. Ca-channel blockers, catecholamine-depleting agents (eg, reserpine), clonidine, digitalis/digitalis glycosides, insulin & oral hypoglycemics. May decrease the hypotensive effect w/ prostaglandin synthetase inhibitors (eg, ibuprofen, indomethacin). May counteract the effect w/ sympathomimetic drugs (eg, epinephrine).
